We are continuing to explore the Professional Practice Model (PPM) and how it supports our practice at Houston Methodist Sugar Land hospital (HMSL). Within the PPM is the Patient Care Delivery System (PCDS). This is depicted as the first wheel of the PPM containing three arrows and represents the structures and processes that promote continuous, consistent, efficient and accountable patient and family centered care. The staff nurse leader drives collaboration and coordination of patient care with the patient, family and interprofessional team. Patient and family centered care is based on the core concepts of Dignity and Respect, Information Sharing, Participation, and Collaboration. Coordination and collaboration both require excellent communication skills among nursing, interprofessionals, patients and families in order to achieve excellent outcomes. Be mindful of how you are providing patient and family centered care, no matter your profession. The core concepts of patient and family centered care are aligned with our I CARE values and provide a framework for us to improve our patient care.
