July 24, 2017: Patient and family centered care is our care delivery system here at Houston Methodist Sugar Land Hospital. The patient and family are the center of everything that we do. When the structures and processes of patient and family centered care work as designed, they yield high quality outcomes. These outcomes are displayed on the second wheel of the Professional Practice Model and include, Safety, Quality, Knowledge, Efficiency, and Satisfaction. Magnet organizations are focused on the outcomes of care. Reflect on how collaboration with other professionals to provide patient and family centered care has yielded excellent outcomes.
