Stop the Line – In Action

We want to recognize Sara Means, CT Tech, for stopping the line. She protected patient safety by correctly stopping the line when two patient identifiers didn’t match.

Another example of a recent Stop the Line story was last week when a patient’s blood was received for a nuclear medicine study.  Upon packaging the blood to be sent off and tagged with radioactive isotope, Vallon Gray noticed the patient”s name did not match the sticker.  Carri Clark assisted Vallon in troubleshooting the mislabeled blood. They determined that for patient’s safety, the patient should be re-drawn and the original blood discharged.

Thank you all for a job well done to protect patient safety!

As a reminder, Stop the Line is a process that empowers all employees and physicians to speak up and take action when patient safety may be at risk.

PCPS 138 Stop the Line: Responsibility and Authority to Intervene to Protect Patient Safety
