Houston Methodist Sugar Land Hospital has been recognized by Institute for Healthcare Improvement (IHI) as an Age-Friendly Health System – Committed to Care Excellence. This recognition signifies that the hospital is leading the way in making sure that every day, every older adult receives age-friendly health care. Houston Methodist Sugar Land is the first hospital in Fort Bend and among the first hospitals in the country implementing Age-Friendly health care.

The four essential elements of an Age-Friendly Health System Hospital are the “4M’s”:
- What Matters: Know and align care with specific health outcome goals.
- Medication: If medication is necessary, use Age-Friendly medications.
- Mentation: Prevent, identify, treat, and manage delirium.
- Mobility: Ensure that older adults move safely every day and prevent falls.
The HMSL Senior Wellness Committee has worked diligently to ensure that 4M’s care is delivered at HMSL. With the support of Janet Leatherwood, CNO, VP, this interdisciplinary committee is led by Tricia Lewis RN, Mini Manavalan RN, Charlie Tran, Gordana Jasmak PharmD, Megan Thibeaux PharmD, and Julie Pickle, PT.
All older adults deserve safe, high-quality health care and the HMSL Senior Wellness Committee strives to improve outcomes for older adults every day through outstanding nursing care, volunteer visits, pharmacists and therapy interventions all focused on great care for older adults.
