MRI Screening Form Process

Inpatient only, including ER.

In an effort to minimize medical errors, a new MRI screening form process will be implemented.

We are asking nurses to help patients fill out the existing screening form. The nurse will verify that the patient has signed the form and that all the information is correct.

The nurse will then fax the completed form to 713.790.8781 and an MRI technologist will verify and initiate coordination with the nurse to the MRI department for testing.
