Year End Tax Forms — W-2 and 1095-C
You’ll receive two important tax forms from Houston Methodist — your W-2 by the end of January and your 1095-C by March 2. We encourage you to sign up to receive your forms electronically instead of having paper copies mailed. It saves time and money!
If you choose to receive your W-2 online, it will be available in mid-January. If you don’t elect an electronic W-2, you will receive it at the home address you have in MARS by Jan. 31. Click here to change your address in MARS.

What do I do with my 1095-C form?
Although you’ll need the W-2 form to file your federal tax return, you can file your 2018 federal tax form without attaching a 1095-C form. When you receive your 1095-C form, keep it with your tax records should you need to provide proof of medical coverage to the IRS. The 1095-C is required by the Affordable Care Act to document that you and your dependents had qualifying health coverage in 2018.
