Case Management and Social Work Director

Please welcome Angela Chisholm as the Director of Case Management and Social Work. She comes from the Temple area where she was the Regional Director of Case Management for a 600-bed hospital. She holds a bachelor’s degree in Nursing and a master’s in Health Care Management.

Angie has worked for the state of Texas as a State Surveyor, as well as other state agencies, including the department of insurance. She has also worked in various nursing roles in the outpatient and inpatient settings including care management. She states her most important role at present is “Grandma” to her 4-year-old granddaughter, Aliyah.
